Code Review
An extensive code review is in progress in the frame of the licence modification for OTB (from CeCILL to Apache). I can provide the link to Landon to follow the progress (work in progress).
Moreover CNES commissioned Black Duck to perform a code review of the library in 2011. The report points only 6S which is publicly available but without license (its authors gave a special permission to Orfeo Toolbox team ton include it). I don't think that I can make the report public.
